You may also be asked for the company names and addresses of any other business you worked for in the past 2 years. You also need their Employer Registration Number. If you worked for the federal government or were in the military, additional forms may be required.

You’ll need to provide your former wages so your benefits can be calculated. The amount of benefits you receive depends entirely on what state you live in. Some states are very generous and will provide half of your old income in the form of unemployment benefits, but a smaller percentage is also common. Many states now allow people to receive unemployment benefits through direct deposit, so it’s also important to have your bank routing number on hand when you apply.

For example, in New York state all people receiving unemployment benefits are required to fill out a Work Search Record. This record can be kept in paper format, or saved on the New York State website. The state will periodically view your Work Search Record to ensure that all Work Search Requirements are met.

Although not every state has the same Work Search Requirements, it’s a good idea to keep clear and full documentation of your search for work while collecting unemployment. You’re required to be searching for a new job while collecting unemployment, and in many cases states can request information about your job search for up to a year after your benefits have stopped. Therefore it’s important to be prepared.

Applying for unemployment can seem daunting at first, but by getting your documents in order and knowing what to expect, the process can be greatly simplified. Many people find that the process is not nearly as difficult as they thought.
